Gateway Manor Community Residence

GatewayManorThe Gateway Manor Community Residence provides the opportunity for 14 mentally ill individuals to live in a family setting while developing the skills needed to fulfill the ultimate goal of independent living. The residence is certified by the Office of Mental Health and has a 24-hour professional staff on dutyThe fully accessible, one-story contemporary arts and crafts style residence provides a home-like environment and is located in an attractive, country environment outside of the city of New Paltz. The residence has a large property and takes full advantage of the nature and environment. Transportation is provided to either New Paltz. Kingston or other community services as necessary..

 
Gateway Manor is a transitional residence with a 2 year average length of stay.
Eligibility Requirements: 
•   Persons age 18 or over
•   Primary diagnosis of mental illness
•   In need of supervised living, but possesses the ability to gain skills needed for more independent living
•   Each applicant will be interviewed prior to admission to determine if the program is appropriate for his/her needs
•   No person will be excluded from the program on the basis of race, religion, color, sex, or national origin
 
Services Provided:
•   Training & supervision in the activities of daily living
•   Counseling
•   Crisis management
•   Medication management
•   Socialization opportunities
•   Room and board
•   Transportation
•   Referral to and coordination with appropriate health, mental health, rehabilitation, and supportive services
 
Service Planning:
Each individual participates in developing his/her own Individual Service Plan, which identifies training and supervision needed to address functional limitations. Typically, individuals require assistance in developing:

•   Interpersonal skills

•   Employment potential
•   Ability to carry out activities to live independently
•   Nutrition and personal care skills
•   Cooperativeness, problem solving skills
•   Self-medication skills
•   Managing psychiatric symptoms
 
Fees:  
The fee is based on SSI Level II Residential Care and Medicaid Restorative Service rates for community residence and is subject to change.
