Supported Apartments Program


background-city-facadeThe Gateway Supported Apartments Program provides support to mentally ill individuals as t
hey strive to successfully integrate into independent community living in the housing they have chosen.The participants play an active role in identifying their housing needs as well as actively pursuing the skills needed to maintain their preferred long term housing.

The Supported Housing Program has no expected  length of stay. Residents are supported to obtain public entitlements to further their independence with the goal of future independent community housing.
Supported housing apartments are located in a variety of communities in both Dutchess and Ulster Counties.  Residents are encouraged to participate in the selection process.  Apartments are furnished by Gateway and upon successful program participation ( one year or more ) we will turn over the security deposit and all furnishings to the resident at time of discharge.

Eligibility Requirements: 
   Persons age 18 or over
   Primary diagnosis of mental illness
           Some programs may require homeless status, MICA diagnosis,VA or parental status

   In need of supervised living, but possesses the ability to gain skills needed for more independent living
   Each applicant will be interviewed prior to admission to determine if the program is appropriate for his/her needs
   No person will be excluded from the program on the basis of race, religion, color, sex, or national origin

Services Provided:
   Assistance in Obtaining Housing
Financial Assistance

Start up funds for:
One time rent and utility security deposits and phone installation fees and house furnishings such as furniture, bedding, kitchen equipment, etc.

Rent stipend to assist meeting rent and utility obligations.All individuals are responsible for paying a portion of the rent based on individual income levels.

Support Services
A.   Prior to placement:
   Processing incoming referrals
   Assist in identifying  housing need
   Assist in housing search
   Assist in lease negotiation
   Assist in purchases for apartment set-up and move in
   Establish budget and payment schedule
   Development of Individual Support plan prior to move in
B.  After move in:

   Emergency services
   Home visits as per Support Plan
   Ongoing case management and advocacy
   Brokerage between tenant and landlord

Service Planning:

Each individual participates in developing his/her own Individual Service Plan, which identifies training and supervision needed to address functional limitations. Typically, individuals require support in developing

   Interpersonal skills
   Employment potential
   Ability to carry out activities to live independently
   Nutrition and personal care skills
   Cooperativeness, problem solving skills
   Self-medication skills
   Managing psychiatric symptoms
   Training & Supervision in A.D.L.’s
   Crisis Management
   Medication Management
   Socialization Opportunities
   Room & Board
   Referral to and coordination with appropriate health, mental health, rehabilitation and supportive services.


The fee is based on SSI Level II Residential Care and Medicaid Restorative Service rates for community residence and is subject to change
