Care Coordination

Care Coordination is a person-centered service designed to assist individuals who have a mental health diagnoses navigate and connect to providers, resources and tools in the community that may help in maintaining recovery and self-sufficiency.

Care Coordinators are trained, experienced professionals who can help one develop a plan of intervention, meet with one regularly to assist in addressing goals and they can advocate for one with various providers throughout Ulster County. Some challenges/barriers that Care Coordinators can help with are:

  • Transportation
  • Educational
  • Vocational  
  • Medical
  • Residential
  • Social Connectedness
  • Case Management
  • Relapse Prevention
  • Legal

Eligibility

Adults aged 18+ who reside in Ulster County and are eligible to receive mental health services.
